Some forms require a Customer Code or Account Reference number. This can be found on the first page of any bill related to that line or service.

SEPA Direct Debit Form

Download and fill in this form to apply for Direct Debit using your local IBAN. Submit the form to us at directdebit@epic.com.mt. We’ll get back to you with confirmation once we’ve processed it.

Bank Card Direct Debit Form

Download and fill in this form to apply for Direct Debit using your credit card. Submit the form to us at directdebit@epic.com.mt. We’ll get back to you with a secure link for you to submit your credit card details to us.

Direct Debit Cancellation Form

To cancel your Direct Debit for your own account, fill in this form and drop it off at any Epic store. Please also present your ID card or passport.
