Authorisation Form

To authorise another person to make a specific change to your Epic account or line, fill in this form and tick the relevant box for change you want to make.
The second person must present the form and a copy of your ID or Passport at a Epic store together with their own ID card or passport to make the required changes.

Insurance Letter

If you have lost your phone and would like to claim it from your insurance company, visit a store with your ID card or passport so we can provide you with an Insurance Letter.
