Bank Card Direct Debit Form

Download and fill in this form to apply for Direct Debit using your credit card. Submit the form to us at directdebit@epic.com.mt. We’ll get back to you with a secure link for you to submit your credit card details to us.

Direct Debit Cancellation Form

To cancel your Direct Debit for your own account, fill in this form and drop it off at any Epic store. Please also present your ID card or passport.

Transfer of Line Form

To transfer your Pay Monthly line to another person, fill in this form. Both the current owner and the new owner must visit a Epic store with their ID cards or Passport and this form.
